Eligibility

You may be eligible to receive services at the Community Health Care Clinic if you:

• Are a resident of McLean County
• Not receiving Bloomington Township medical assistance
• Not have any access to affordable health insurance
• Not qualify for any of the following: Public Aid Medical card, Veteran's benefits, Medicare, Medicaid, Disability or All Kids
• Have a TOTAL gross income fall within 185% of Federal poverty guidelines

You must be apple to show proof related to all of the above.
