The new Food Farmacy program increases healthy food access for patients working to improve their overall health. This program in only possible because of the fantastic partnership between CHCC and Home Sweet Home Ministries. The Food Farmacy program allows CHCC patients with diabetes or severe cardiovascular disease to receive a Food Farmacy voucher from CHCC, much like a written prescription for medication. This "prescription" allows the patients to go to the HSHM Bread for Life Co-op and shop for free fresh produce and other groceries.
